We accept all insurance however we are an Out of Network provider. We will verify your out of network benefits for you and submit to your insurance company. Payment will be due at time of service.
We ask that you follow the personal, confidential link to pre-register prior to your visit. The link allows us to capture your demographic information, credit card on file for payment, and sign all release forms and documentation to bill your insurance and permission for treatment. We want to maximize your appointment with us and limit time spent on paperwork.
